I need to claim medical insurance against the amount paid to fortis, anandpur, kolkata. For this i need the part b (To be filled by hospital) form from the hospital. This is being denied by the insurance desk of the hospital saying "they don't know about it" and "i need to fill this form, hospital is not responsible". This form is about hospital details like registration number of the hospital and patient diagnosis details including the doctor registration number. How a patient is supposed to fill such information. After asking for multiple times, still no response from hospital.
