Amri Hospitals — mistake in discharge summary medicine prescription

This is a complain against the person who had prepared the 'discharge summary' of my father mr. Jayanta kumar ghosh (Aged: 71yrs) who was admitted in amri dhakuria, kolkata for an avf from 15/04/2018 – 16/04/2018. He is a ckd, hypertension, highly diabetic and heart patient. Due to the avf dr. Siddhartha mukherjee had prescribed him few medicines to be taken for 5 days post discharge. But in the 'discharge summary' out of these medicines for one particular antibiotic (Lizolid tablet) no duration was mentioned. Hence my father had continued taking that antibiotic after 5 days also. He started feeling very weak, nausea and vomiting tendency from the 7th day post his discharge. He has become severely ill now. And is unable to get up from bed. Today when we called up dr. Siddhartha mukherjee he told us that the antibiotic was only meant to be taken for 5 days and so my father should stop the antibiotic immediately. Hence i would like to complain against the person who had actually prepared the 'discharge summary'. Due to his mistake (Not mentioning the duration of taking the antibiotic) my father has become very ill. Such a reputed hospital like amri should not neglect this type of blunder and should take action immediately against the person who had prepared the discharge summary. It is a matter of my father's life after all. Such a life-threatening incident is not at all acceptable from amri dhakuria hospital staff.
